DNP Clinical

All clinical clearance documentation requirements are due 14 days prior to the start of the clinical course. If a student is not cleared 7 days prior to the start of the first day of their clinical course, they will be administratively dropped without warning (NO EXCEPTIONS).

REQUIREMENTS PRIOR TO CLINICAL COURSES - All students need to have completed:

1. Submission/approval of Student & Preceptor Information Request Forms
2. Completion of signed Confirmation of Clinical Placement Form - Signed & returned by preceptor
3. Completed contract if required by agency. Signed by facility & university
4. Health clearance through EXXAT - Student needs to be cleared by EXXAT with submission of COVID vaccine/religious exemption*
5. Completed Urine Drug Screen - Result of "APPROVED" through Universal
6. Completed Background Check - Result of "APPROVED" through Universal (Anyone who lives in New York State or who has lived in New York State in the last 7 years must complete a New York background check in addition to the one in New Jersey. A student may also be requested to complete a NYS background check if an alert comes up even if they have not resided or worked in NY in the last 7 years.*The background recheck should only be completed when the initial background check is about to expire or has expired*

                                                                                                        

*Vaccine exemptions must be submitted on official physician letterhead and include the student’s full name. The practicum site must also provide an official letter on its letterhead approving the exemption before clinical hours begin. All required documents must be uploaded to Exxat and receive formal approval prior to the start of the clinical experience.
