All faculty, librarians and professional staff who intend to request a leave of absence (with or without pay) must make a formal written request to the President. A copy of this request should be provided to the department chairperson, dean or director and appropriate Vice President.
Any request for a leave of absence (including paid sick leave) for a period of four weeks or more must be formally approved by the Board of Trustees.
Requests for leaves of absence due to illness must be accompanied by documentation from a physician. Necessary medical documentation should include a diagnosis and expected return to work date.
