Each candidate must complete this application and include documentation supporting the application. An outline of prerequisites are on the back of this form. Students must also complete and turn in a Technical Standards for Admission form and a Standard Physical Examination Medical History form with the application.

1. Applications are due Wednesday, November 5, 2014 by 12:30 pm for spring semester admission to the Professional Preparation Phase.  

2. Applications are to be submitted to:

3. This application is to be completed by all students who wish to be admitted to the Professional Preparation Phase. Admission to the Professional Preparation Phase cannot be gained without applying for admission and completing all admission requirements.

4. Prerequisites for Professional Preparation Phase admission (Program Director use only):

______  Completed Athletic Training Program Application turned in to the Program Director by the posted deadline

______  24 credit hours completed for admission

______  100 observation hours at WPUNJ recorded by the applicant and signed by the faculty/staff athletic trainers

______ Overall 2.5 grade point average at WPUNJ

______  Grade of C- or better in BIO 1180 Anatomy and Physiology I

______  Grade of C- or better in BIO 1190 Anatomy and Physiology II

______  Grade of C- or better in ATEP 1500 Introduction to Athletic Training

______ Successful completion of ATEP 2000 Sports Emergency Care

______  Successful completion of ATEP 2400 Prevention and Care of Injury and Illness in Athletic Training taken at WPUNJ in the fall semester of application

______  Current certification in Professional Rescuer CPR with AED

______  Current certification in Advanced First Aid or equivalent

______  Successful completion of Athletic Training Program Standard Physical Exam

______  Completed Technical Standards for Admission form turned in with the application and required materials

______  Completed Student-Athlete/Athletic Training Student Policy form (if applicable)

______  NATA student membership number to the Program Director before the first day of spring semester classes

Students will be informed in writing of admission decisions after the fall semester grades are available.

Candidates who unconditionally meet all entrance prerequisites will be accepted into the ATEP Professional Preparation Phase, provided that the student to clinical instructor ratio as recommended by CAATE is not exceeded.  In the event that this ratio is exceeded, admission into the ATEP Professional Preparation Phase will be limited to those eligible candidates who possess the highest GPA and the greatest potential to excel as athletic trainers as determined by their athletic training observation experience and the results of an interview.

Students not meeting the final entrance criteria at the time of the decision by the Athletic Training Education Program Admissions and Retention Committee will be denied admission to the ATEP Professional Preparation Phase, but the students are encouraged to apply for admission the following fall semester.
